- provides 24-hour residential care and supportive services for youth with a history of trauma and the presence of complex needs. The residential sites throughout NYC and Long Island locations serve a variety of needs including a Specialized Diagnostic Center, Qualified Residential Treatment Programs and Residential Programs for Pregnant and Parenting youth. These residential programs provide youth with structure and supervision designed to provide evidence-based treatment to promote emotional and physical safety and stability.
POSITION SUMMARY:
The Family Team Conferencing Facilitator is responsible for working the residential team to schedule and lead the Family Team Conference that engages members of the family, and their natural support in discussions related to safety, well-being and plans for permanency. The Family Team Conference Facilitator is responsible for facilitating and tracking all agency-led conferences, utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each child and family is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders.
REQUIRED QUALIFICATIONS:
- Master’s degree in Human Services field required with 2 years of experience in casework within ACS programs REQUIRED.
- Prior experience leading group sessions preferred.
- Prior experience working with children and families known to the Child Welfare system, impacted by Abuse and Trauma preferred.
- Strong Engagement, Organizational and Communication skills.
- Knowledge of and commitment to child welfare best practice standards.
- Understanding and willingness to commit to the agency’s mission, core values, Sanctuary commitments.
- Ability to accommodate a flexible work schedule, including evenings.
- Proficiency in Microsoft Office suite
- Demonstrate competency in child development.
- Demonstrate cultural competency.
RESPONSIBILITIES:
- Facilitating community-based, home-based, virtual Family Team Conferences with youth and families in ACS-funded residential programs (QRTPs, YPPP, Diagnostic programs)
- Maintain compliance and tracking of timely scheduled and held Permanency Family Team Conferences.
- Maintain fidelity to the FTC model, including establishing an atmosphere conducive to strength-based, solution-focused engagement of all stakeholders.
- Ensuring the safety and well-being of each child is assessed through a strength-based lens.
- Utilizing strong engagement skills to yield positive safety and permanency outcomes.
- Engages and facilitating critical discussions with participants in development of plans.
- Responsible for maintaining case documentation and data entry into systems of record: Connections, ASARA.
- Participation in weekly supervision, program meetings, trainings and workshops to enhance professional development.

Salary Range
$55,000-$58,000
