MercyFirst is currently seeking a Aftercare Caseworker.

Some Remote Work Negotiable for this Position!

EDUCATION, LICENSURE, AND/OR CERTIFICATION REQUIREMENTS: MSW Degree in Social Work or related field. Must maintain a valid NYS Driver’s License with a satisfactory driving record.

EXPERIENCE REQUIRED: 1+ years work related experience with the knowledge and training in working with children and families including child development stages and family dynamics.

Brief Outline of Essential Tasks

• While youth is in placement: minimal monthly meeting with youth and family.
• Meet with youth 2x weekly/legal guardian weekly for the first 6 weeks; once weekly thereafter.
• Collateral contact weekly with youth’s school/therapist/other resources.
• Conduct home assessment within first week of placement with youth’s Caseworker.
• Utilize Solution Based Casework.
• Participate in rounds: record notes and provide updates on youth.
• Write and update psychosocial assessments.
• Rotate schedule of family visiting.
• Participate in red flag meetings.
• Participate in DSS SPR meetings.
• Host monthly Parent Support group.
• Identify community-based supports (for both youth and legal guardian) prior to discharge.
• Attend all court appearances with youth while in aftercare and provide a court update.
