AGENCY BACKGROUND:

MercyFirst is a not-for-profit human and social service agency that has been serving children and families in need since we were founded by the Sisters of Mercy/Hermanas de las Misericordia in 1894. Today our agency continues to address the emotional and physical needs of children and families in Brooklyn, Queens and across Long Island through innovative treatments and life-changing interventions. We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. Each year, MercyFirst serves more than 3,000 children, teenagers and families overcome enormous obstacles, re-imagine their futures and develop their full potential.

PROGRAM BACKGROUND:

The MercyFirst Brightwaters Group Home has been designed to be a residential program providing care for up to 12 adolescents, between the ages of 14 up to 20, at any given time. The group home offers a community-based, home-like atmosphere for youth deemed to need residential foster care placement, providing a structured, supervised and therapeutic environment to meet their special needs. The program is designed for individuals with a history of behavioral difficulties that cannot be successfully maintained in their family home setting or a foster home, a history of multiple unsuccessful placements, as well as victims of abuse/maltreatment and trauma. The program provides individual, group, and family therapy with 24-hour supervision within our structured therapeutic milieu.

 

Admission to the RISE program will be determined by Family Court recommendation when incarceration is deemed to be too restrictive of a setting. Intensity and frequency of service plans will be determined by matching services to the individualized needs and risk of the youth. This approach will be utilized for adjudicated delinquent youth who are 16 and 17-years old at the time of committing the offense. The MercyFirst RISE program will serve up to ten male and four female youth, residing in designated cottages on the Syosset campus


SUMMARY:

Under the Supervision of the Sr. Director of Long Island Residential Programs, the Aftercare Caseworker will provide the youth and their family with continued casework support in the home after discharge, up until the youth's placement order expires, to ensure successful reintegration into the community and in the home.
 

REQUIRED QUALIFICATIONS:

  • MSW or Masters in related field.
  • Prior experience working with adolescents required, prior experience with at risk youth strongly preferred. Prior Casework experience required.


RESPONSIBILITIES:

  • While youth is in placement: minimal monthly meeting with youth and family.
  • Meet with youth 2x weekly/legal guardian weekly for the first 6 weeks; once weekly thereafter.
  • Collateral contact weekly with youth's school/therapist/other resources.
  • Conduct home assessment within first week of placement with youth's Caseworker.
  • Participate in weekly rounds: record notes and provide updates on youth.
  • Write and update psychosocial assessments.
  • Participate in red flag meetings.
  • Participate in DSS SPR meetings.
  • Complete monthly discharge plans
  • Identify community-based supports (for both youth and legal guardian) prior to discharge.
  • Attend all court appearances with youth while in aftercare and provide a court update.

Salary Range:
$58,000 - $60,000 per year
Hiring bonus of $1,000 after 500 worked hours
